FSM's shall update the SIRS on-line spreadsheet monthly or as needed, for compilation and analysis.

This system is designed to track all Safety Incidents occurring to NOAA employees.

All reportable incidents occurring since 10-01-01 shall be entered into SIRS.
Beginning in June 01, 2002 all incidents with Case Types 1, 2, and 3 shall be included, if information is available.

It is designed to not only track all "Reportable OSHA" incidents, those where employees are injured or have a work related illness that results in worker compensation forms CA-1 or 2, to be completed, it will also track the following classes as follows:

Near Misses An incident without injury but that could have resulted in injury if an employee had been directly involved.

Minor An incident involving an employee with a scratch type of injury requiring no medical attention.

First Aid An incident where simple first aid treatment is provided, such as a band aid or a one time dose of aspirin, etc. This may or may not involved a medical practitioner.

Reportable Only This involves medical treatment without modified work or lost time, such as closing of a cut with a stitch, getting a prescription filled or any simple medical treatment.

Reportable Modified Work This involves a medical injury that has limited duties prescribed by the physician, such as limited lifting, walking with a cane, or any modification of the job.

Reportable Lost Time This involves an injury that is of a serious nature to require a full day lost after the time of the
incident.

Reportable Fatality Self explanatory.

Property Damage
(no injuries)
This includes damages to motor vehicles, equipment, facilities, and other NOAA related property.
